Before receiving an award disbursement, MA Repay participants must complete the Eligibility Verification Form with their current loan information and provide statements to verify eligible outstanding loan balance. You will receive an email notification with instructions when it is time to complete the Eligibility Verification form. Guides to assist you in finding the required information are available to the right for the most common loan servicers.
